No other votes for these I would imagine but I have a soft spot for Heaven and Hell and Mob Rules. I had these on a C-90 when I was a kid and played them to death. Much as I've come to appreciate the original Sabbath I'm afraid its those two which are burnt into my brain.
The thing is that I've spent almost all of my Sabbath time listening to We Sold Our Souls For Rock and Roll, so my sense of the albums is pretty much nonexistent. However, I will note that two of my favorite songs, "Snowblind" and "Supernaut" are on Vol. IV, which makes it a plausible contender.
holy shit jess, great question! my favorite sabbath album has got to be masters of reality. that first song, sweet leaf, which begins with coughing, is one of the best hard rock songs ever. i used to smoke very heavy dope and trip on that album for hrs.
my favorite song? n.i.b. with geezer butler's deep bass leading the beats.

Paranoid, by far. Although, as far as paranoia albums go, I'd say Pink Floyd's Animals takes the proverbial cake.
